Welcome to the “perfect storm” of healthcare! This might be a description for all of healthcare in America, but there are potentially earth-shaking implications for our older adults and their health care team. The interwoven complexities generated by our rapidly changing healthcare system are daunting. The health care professional may not feel prepared to face the new challenges presented by the new payment models that are emerging, the emphasis on quality measures, the effects of increasing life expectancy, and the increasing prevalence of comorbid chronic diseases and the need to better identify high-risk patients.
We will attempt to analyze the journey older adults might experience moving between multiple settings of care—home to hospital to nursing home and back again. Each of these moves if improperly handled can lead to unwanted complications and care that is less efficient, more costly and poorer quality of care. The goal of this conference is to seek ideas how to intervene more proactively to help care providers better understand our patients’ needs as they navigate the healthcare system. We hope to provide insights into the changes that are occurring in healthcare and look for ways to promote safer care and more effective care at the right time and at the right place. We will attempt to find ways to identify the high risk patients that require additional specialized resources so that we can provide more appropriate, affordable patient-centered quality care. Lastly, because dementia in particular stretches the limits of affordable healthcare, we will examine better means to identify and better care for the cognitively impaired patient and their families.
This conference is meant to enhance the knowledge base and skills of physicians, nurses, pharmacists, psychologists, dietitians, allied health and rehabilitation therapists, social workers, health facility administrators, and other interested health care professionals.

Conference Objectives
• Recognize the role of changing Medicare payment systems in the care of older adult.
• Delineate patient characteristics that predict higher risk of healthcare related complications.
• Identify ways in which to more efficiently transfer patient information between healthcare settings.

Nursing: St. Vincent Hospital and Health Care (OH-435, 12/1/2016) is an approved provider of continuing nursing education by the Ohio Nurses Association (OBN-001-91), an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Approved provider status does not imply endorsement by the provider, ANCC or ONA of any products displayed in conjunction with this activity.
This program will provide the participant with 6.0 Continuing Education contact hours.
Pharmacy: St. Vincent Hospitals and Health Services Pharmacy Department is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Activity Number (UAN) to 0556-0000-14-051-L01-P Application Activity. This program is approved for 6.0 Contact Hour CE (0.6 CEU). Active participation in Case Studies/Panel discussions and an evaluation of speaker and program is necessary for continuing education credit to be given. Statements of credit will be issued via CPE Monitor.
Indiana State Board of Health Facility Administrators: This program has been approved by the Indiana State Board of Health Facility Administrators to present the continuing education program for 6.0 hours.
National Association of Social Workers: In partnership with the St. Vincent Medical Social Services Department—an approved CEU provider by the Indiana Professional Licensing Agency--The Indiana Social Worker, Marriage and Family Therapist and Mental Health Counselor Board has approved this organization to provide 5.50 Formal Category 1 Continuing Education for LSW, LCSW, LMHC, and LMFT. However, the licensees must judge the program’s relevance to their professional practice. The CEUs are provided through the license of St. Vincent Medical Social Services.
Indiana State Psychology Board: This program has been approved by the Indiana State Psychology Board to present the continuing education program for 6.0 hours.
Indiana State Board of Occupational Therapists: This program has been approved by Indiana State Board of Occupational Therapists to present the continuing education program for 6.0 hours.
Physical Therapy: This program is pending approval by INAPTA to provide six hours of continuing education.
Allied Health Care Workers: Those attending the entire day will receive a certificate of attendance of 6 hours.
Academy of Nutrition and Dietetics: This program has been submitted for review and approval by the Commission on Dietetic Registration to provide 6.0 CPEU’s.
Case Manager: This program is pending approval by The Commission for Case Manager Certification to provide continuing education credit to CCM Board Certified case managers.
Nurse Practitioners: This program is pending approval by the American Association of Nurse Practitioners. This program was planned in accordance with AANP CE Standards and Policies and AANP Commercial Support standards.
After participating in this program, a program evaluation must be completed for continuing education credit to be given. Participants must attend 80% to receive credit and should claim only the credit commensurate with the extent of their participation in this activity.
